- Any information forms we have previously given to you
- Your insurance cards (we need to obtain the insurance subscribers name, S.S.# and D.O.B. to submit insurance claims)
- Copay
- Any outstanding balances
- Notify the staff if your address, phone numbers or insurance has changed
- Notify the staff if you need to update/change your HIPAA privacy information including who we can share your health information with
- All current medications including dosages
- Questions/concerns you would like to be addressed at your visit
